Boston Emergency Medical Services (BEMS) is one of the nation's oldest providers of pre-hospital care, tracing its beginnings back more than 100 years, when the department was known as the City Hospital Ambulance Service.  BEMS is a third service answering to the mayor of Boston through the Public Health Commission.  Since 1892, when its first patient was transported, BEMS has continually evolved to meet the ever-changing health care needs for the City of Boston.  

BEMS provides pre-hospital emergency care for more than 600,000 night time and 1.2million daytime residents and workers of Boston.  The 1,700 Boston Fire first responders and 300 emergency medical technicians and paramedics of BEMS respond to approximately 100,000 calls and transports 65,000 patients each year.  The low paramedic to population ratio ensures significant experience for each individual paramedic allowing the practice of highly sophisticated procedures such as rapid sequence intubation and STEMI triage and recognition.  BEMS is also responsible for staffing and maintenance of the medical dispatch for the City of Boston 911 call center.

 Boston EMS is responsible for providing medical support and training exercises in emergency preparedness.  BEMS also has a close relationship with the DelValle Institute for Emergency Preparedness.  The institute, directed by Dr. Brinsfield, is a leader in providing education and practical training for the management of potential WMD and HAZMAT incidents.

Delivery of optimal care is dependent on knowledgeable physician participation and supervision at every level of the EMS system.  BEMS physicians play a vital role in the ongoing design, operation, evaluation and revision of the EMS system from initial patient access to definitive patient care. 

The EMS Fellowship was created at Boston EMS to provide physicians with the necessary experience in operations, training, research and administration to contribute to EMS systems in a variety of settings.  The EMS fellowship is two years in length and includes a Master’s in Public Health at Boston University School of Public Health.  The fellow also has a part time clinical appointment at Boston Medical Center and Boston University School of Medicine.   Applicants for the BEMS Fellowship should be graduates of a 1-4 or a 2-3-4 emergency medicine residency program as they will be responsible for clinical teaching of residents in the Boston Medical Center 2-3-4 program.  Graduates from a 1-3 year emergency medicine residency program with additional experience will also be considered.
